This portal helps you provide your medical history, receive instructions about your upcoming surgery, choose your anaesthesia options, and stay informed with reminders and guides to prepare for your surgery.

You will receive an SMS and/or email from your doctor with a link. Click the link, verify your identity using a One-Time Password (OTP) sent to your mobile, and you’ll be guided into the portal.

You will be asked to: Fill in your personal and medical history. Upload any required medical reports. Choose your anaesthesia option (if multiple options are available). Confirm your understanding of pre-surgery preparations.

You can select the [Continue Later] button. You’ll receive a link via SMS to continue from where you left off.

Yes, you can edit your survey responses after authenticating again with OTP until you submit the form. Once submitted, edits may require contacting your doctor.

If no response is detected within 30 minutes, a reminder SMS with a link will be sent to you. Please complete it promptly to avoid surgery delays.

Click the Help/Chat button on the screen. You can choose a category (e.g., Technical Issue, General Query) and submit your question. Our administrator will reply to you via email.
